We work closely with providers to ensure the best possible outcome for your patient is achieved, and routinely assess compliance and adherence to medication therapy.
Communication
We will communicate with you regularly, providing monthly follow ups with your patients. A pharmacist will review the medication profile, update it, and provide you with a faxed copy of our assessment of the patient’s progress and lab work. In addition, you will be notified in the event of a patient experiencing side effects that require a follow up, or in the event a patient is not adhering to the medication regimen you have prescribed.
Education
Patients will receive one on one counseling with a pharmacist, educating the patient on their specific disease or condition, and going over the prescribed medication therapy and potential side effects. We will emphasize the importance of compliance and adhering to the prescribed therapy.
